Role Overview:
Cal-AIM Housing Navigators play a crucial role in supporting the agency's mission, vision, and philosophy. Their primary responsibilities include providing housing transitional navigation services, housing deposits, tenancy, and sustaining services for members authorized by Managed Care Plans (MCPs). They also offer Community Supports housing services and collaborate with local Continuum of Care (CoC) homeless service systems, MCPs, and Enhanced Care Management (ECM) providers to ensure high-quality services for members. Housing Navigators report to the Cal-AIM Program Manager of Housing Navigators and must have experience in the housing and homeless field, as well as knowledge of Housing and Urban Development (HUD) funding and regulations.
Responsibilities and Duties:
- Outreach and Engagement:
- Collaborate with team members to develop outreach strategies for isolated consumers.
- Participate in community presentations to introduce the Cal-AIM program to partners and potential consumers.
- Consumer Support:
- Demonstrate commitment to consumer autonomy and empowerment.
- Build respectful interpersonal relationships with consumers.
- Assist consumers in identifying barriers and accessing appropriate healthcare resources.
- Support members in accessing education and employment services.
- Program Participation:
- Increase awareness of self-help programs and consumer advocacy organizations.
- Encourage members to leverage their talents and skills.
- Attend staff meetings to contribute to program development and training.
- Service Delivery:
- Conduct intakes and assessments to determine housing needs.
- Develop individualized housing support plans based on assessments.
- Identify and secure resources for affordable housing and rental subsidies.
- Assist with housing applications, pre-inspections, and landlord/tenant mediation.
- Educate clients on landlord/tenant rights.
- Maintain documentation and housing charts according to agency standards.
- Client Support:
- Facilitate access to community resources to prevent eviction.
- Assist with benefits advocacy, including SSI eligibility and application support.
Other Duties and Responsibilities:

Qualifications:
- Minimum Education and Experience:
- Bachelor’s degree or related experience in social services, or 3 years of combined experience in case management, housing supportive services, and working with the homeless population.
- Knowledge of housing rights related to homelessness and low- to moderate-income housing.
- Other Specific Requirements:
- Valid California driver’s license (if driving frequently) or ability to use public transportation.
- Extensive knowledge of local community resources.
- Preferred Qualifications:
- Bachelor’s degree or equivalent experience in a social service-related field.
- Highly Desirable Qualifications:
- Knowledge of Coordinated Entry Systems (CES), local CoC systems, and housing authorities.
- Ability to build partnerships with landlords, realtors, and property management companies.
- Training in Motivational Interviewing, Trauma-Informed Care, Harm Reduction, etc.
- Ability to work effectively with diverse communities and manage multiple demands in a high-pressure environment.
- Excellent communication skills and high tolerance for individuals with complex needs.
